Acute care nurse practitioner [h1.location_city]

CenterWell is looking for a dedicated Advanced Practice Provider who is Board Certified / Board Eligible for an exciting primary care role.

A Day with CenterWell :

See 12-15 patients per day, allowing for personalized patient interactions

Play a crucial role in a value-based care model, boosting patient's quality of life

Handle a diverse caseload from preventive care to chronic conditions.

Dedicate 100% of your time to direct patient care, including patient-facing time and associated administrative tasks

Active, unrestricted NP / PA license in the state

Current NP or PA certification

Current DEA license or the ability to obtain one

Passion for enhancing consumer experiences within an organization

This role is patient-facing and part of Humana / Senior Bridge's TB screening program. TB screening will be required.

Preferred Skills :

Familiarity with Value-Based Care model

2-5 years experience with geriatrics patients in a primary care setting

Autonomy in chronic disease management

Experience in outpatient senior care

Knowledge of HEDIS measures
