  • Support individuals in achieving goals outlined in their Person-Centered Plan (PCP) and Individual Support Plan (ISP).

  • Encourage community inclusion by assisting with recreational activities, appointments, employment, volunteer opportunities, church, and social events.

  • Provide emotional support, mentorship, encouragement, and positive reinforcement.

  • Monitor and document daily progress, behaviors, health concerns, and significant events in accordance with agency policies.

  • Collaborate with Qualified Professionals (QPs), healthcare providers, guardians, family members, and CareLink Solutions staff to ensure quality care.

  • Transport individuals safely to appointments, community activities, and other approved destinations as needed.

  • Respond appropriately to emergencies or behavioral situations while following agency procedures and safety protocols.

  • Maintain confidentiality while protecting the rights, dignity, and independence of each individual served.

  • Compassionate, patient, dependable, and committed to providing person-centered care.

  • Excellent communication and interpersonal skills.

  • Ability to maintain accurate documentation and complete required reports on time.

  • Valid driver's license, reliable transportation, and acceptable driving record.

  • Ability to pass a criminal background check and drug screening.

  • CPR/First Aid certification or willingness to obtain upon hire.
