As a Home Health Physical Therapist, you will provide skilled therapy services to patients in their homes throughout Forest and surrounding communities in Scott County. Your caseload will typically include adults and older adults recovering from surgery, managing chronic conditions, or working to improve mobility, strength, and safety in their home environments. You will perform comprehensive evaluations, develop individualized plans of care, and deliver evidence-based interventions focused on functional independence, fall prevention, and improved quality of life. A typical day includes traveling to patient residences, conducting assessments, implementing therapeutic exercises and activities, performing gait and transfer training, and educating patients and caregivers on home exercise programs, safety strategies, and equipment use. You will document visits using an electronic medical record system and coordinate closely with nursing and other disciplines to ensure cohesive, patient-centered care. This role is ideal for a Physical Therapist who enjoys autonomy, one-on-one patient interaction, and the ability to see the direct impact of their work in patients’ daily lives. You will manage your schedule within agency guidelines to meet productivity expectations while maintaining high-quality care. Visit volumes and patient ratios are structured to support thorough, personalized visits rather than rushed encounters. Shifts are generally daytime hours, with flexibility based on patient needs and agency scheduling practices. Weekend or on-call coverage may be required on a rotating basis, depending on agency policies. You will spend a significant portion of your day in the field, driving between patient homes, and must be comfortable working independently while maintaining close communication with your clinical team.
