Under the direction of the Senior Vice President of Operations, this position is responsible to maintain robust initiatives to ensure annual well visits are completed with emphasis on identifying and addressing all chronic conditions year over year and quality care gaps are identified and addressed. The Practitioner will interact with other departments, clinic personnel, and outside providers in a professional and friendly manner to create and maintain a positive relationship with our internal and external customers.
- Participate in the development and implementation of strategies to identify HCC opportunities.
- See BFMG and CCPN membership (in a face-to-face visit and / or telehealth) for their annual well visit that would include the capturing of all new diagnosis and recapturing prior diagnosis to include proper supporting documentation for each.
- At the direction of the Medical Director provide HCC related clinician comments to the rendering providers in the network.
- See custodial and skilled patients for the purpose of identifying, diagnosing, and documenting all HCCs for a given year.
- Provide Clinical support to the Quality Management initiatives to close care gaps including medication adherence measures.
- Work with the BFMG / CCPN pharmacist to educate patients on the importance of medication adherence and work to identify and address barriers to the adherence.
- Other reasonable duties assigned by Senior Vice President of Operations.
4.1 Be a licensed Nurse Practitioner or Physician Assistant with an unrestricted current California License
4.2 Practiced as an NP or PA for at least one year in a primary care setting
4.3 Working knowledge of HCCs and HEDIS preferred
4.4 Current Basic Life Support (BLS) certification.
The pay range for this position at commencement of employment is expected to be reasonably between $166,400 and $187,200 annually for full-time employment. However, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience.
If hired, employee will be in an "at-will position" and the Company reserves the right to modify base salary (as well as any other discretionary payment or compensation program) at any time, including for reasons related to individual performance, Company or individual department / team performance, and market factors.
