JOB PURPOSE :

The Clinical Nurse Liaison plays a vital role in connecting hospitals and community services with our organization for patient referrals. This position evaluates patients to ensure eligibility for admission to home health or hospice services. The Liaison provides essential clinical expertise to assess how well home health and hospice agencies can meet patient needs. As a sales professional, this individual fosters early and appropriate referrals by building and maintaining business partnerships with facilities and hospitals.

KEY RESPONSIBILITIES :

KNOWLEDGE, SKILLS, ABILITIES :

MINIMUM EDUCATION REQUIRED :

Two (2) years of college or business school, or equivalent combination of experience and training.

MINIMUM EXPERIENCE REQUIRED :

Two (2) years’ experience in home health and / or hospice, healthcare sales / marketing, or other relevant experience.

MINIMUM LICENSURE / CERTIFICATION REQUIRED BY LAW :

Current RN license; RN or LPN graduates with active license.

ADDITIONAL QUALIFICATIONS :

Bachelor's Degree preferred.
