Job description

Job Summary:

The Clinical Outreach Coordinator is responsible for bridging clinical expertise and outreach efforts, ensuring referral sources understand and trust CHC’s services. The Clinical Outreach Coordinator will lead outreach and engagement efforts to position CHC as a trusted resource for mental health services for ages 0-25. A significant portion of this role’s time will be in the community, building relationships with other behavioral health providers and organizations. By engaging partners, fostering connections, and sharing helpful resources, you will play an integral role in expanding CHC’s reach, driving referral growth, and increasing access to care for families in need. This position currently reports to the Clinical Director.

Essential Functions:

1. Provider Relationship Management:

  • Build and nurture collaborative relationships with healthcare providers and community organizations, establishing CHC as a trusted partner in mental health services.

  • Develop and maintain high-touch, collaborative partnerships that create value for referral sources and the families they serve.

  • Serve as a knowledgeable primary point of contact for referral sources and clinical partners, providing timely updates and resources about CHC's programs and availability.

  • Conduct regular outreach meetings, both virtual and in-person, to strengthen partnerships.

  • Maintain commitment to child safety, mandated reporting duty, and professional boundaries.

2. Clinical Outreach and Engagement:

  • Develop and implement strategic outreach initiatives targeting key referral sources to increase awareness of CHC’s mental health services.

  • Represent CHC at community events, health fairs, and professional conferences to raise awareness of our services, in collaboration with CHC’s community outreach team

  • Host events that increase awareness of CHC’s services, including networking events.

  • Deliver presentations, webinars, and workshops that highlight CHC’s clinical expertise and resources.

  • Collaborate with the marketing team to create engaging materials and campaigns tailored to various audiences, including healthcare providers.

3. Data and Impact Measurement:

  • In partnership with the Clinical Director, establish clear objectives and desired outcomes.

  • Track outreach activities and referral trends to measure impact and identify growth opportunities.

  • Provide regular reports on outreach efforts, including metrics on referral volume and conversion rates, to communicate progress and results to CHC leadership.

  • Track all contacts in Salesforce Non-Profit Cloud

4. Internal Collaboration:

  • Work closely with the Care Team to ensure smooth referral pathways and alignment with program capacity.

  • Partner with CHC leadership to identify high-impact community partnerships and opportunities to enhance services based on community needs.

  • Attend Clinical Services staff meeting and other meetings as directed to remain up to speed on current programs, services, and availability.

  • Work collaboratively with other internal teams, including Community Engagement, Marketing, Development, and school leaders, to ensure outreach efforts are coordinated, efficient, and impactful.

  • Work collaboratively with the Clinical Director, Director of Business Operations, Chief Clinical Officer, and other leaders, to manage the outreach budget

5. Budget Management:

  • Manage the outreach budget, in collaboration with the Clinical Director and Director of Business Operations, ensuring resources are allocated effectively and align with strategic goals.

Knowledge/Ability/Skills:

  • Strong interpersonal and relationship-building skills, with an ability to engage clinical and community stakeholders.

  • Strong verbal and written communication abilities, including public speaking and presentation skills and the ability to translate clinical concepts into accessible information for referral sources.

  • A background in psychology, social work, mental health, or a related clinical field, enabling you to speak confidently and deeply about CHC’s clinical services.

  • Ability to learn and understand CHC’s four areas of expertise: ADHD, LD, Anxiety & Depression and Autism Spectrum Disorders

  • Familiarity with mental health, educational, or developmental services, including treatment modalities and the challenges families face.

  • Passion for CHC’s mission and a commitment to serving children and families.

  • Ability to work independently and as part of a team.

  • Excellent time management skills.

  • Proficiency in using Microsoft Office Suite and virtual meeting platforms.

Education/Experience:

  • Bachelor’s degree in psychology, social work, mental or behavioral health, or similar discipline required. Master’s Degree preferred.

  • Familiarity with mental health, educational, or developmental services is preferred.

  • Experience in outreach and marketing is highly preferred.

Physical Requirements:

  • An employee in this position may be required to occasionally lift, move and/or set-up stacks of documents such as marketing materials, lap-top computers, etc. The employee will also be required to work for extended periods of time at a computer.

Special Job Requirements:

  • Hours: Full-time position (.8 FTE equivalent - 32 hours a week), typically Monday through Friday, with some evenings or weekends for events.

  • Location: Based in Palo Alto, CA, with limited flexibility for a hybrid work environment. Outreach activities will require reliable transportation to be able to travel within the local community and surrounding areas
