About the Role
The Roads Home Health Care is seeking an experienced Home Health RN Case Manager to lead patient care coordination for an assigned caseload throughout South Florida.
As a case manager, you will plan, coordinate, and document care for patients from admission through discharge, using your clinical judgment and home health expertise to build individualized plans of care.
This is an excellent opportunity for a registered nurse who enjoys autonomy, patient relationships, and leading a multidisciplinary care team.
