The Norfolk Health District is seeking three (3) contract Community Health Workers (CHWs) to support public health initiatives in neighborhoods disproportionately impacted by the social determinants of health. CHWs will advocate for residents through local initiatives, engage with NDPH staff, and participate in community outreach. These positions are funded under the City of Norfolk's FY2026 Safer Communities Grant Program and will focus on addressing the root causes of violence through health and social services.
Responsibilities :
- Engage residents at hubs, schools, events, and faith-based organizations.
- Encourage participation in civic and community activities.
- Build trusting relationships to support community health needs.
- Facilitate culturally appropriate public health education.
- Assist residents with Medicaid, SSI, SSDI, SNAP, and other applications.
- Identify health resources and challenges within the community.
- Educate on preventive health services.
- Facilitate referrals via the Unite Us platform.
- Maintain strong relationships with NDPH and partner agencies.
- Support strategies addressing social determinants of health.
- Complete accurate and timely documentation.
- Participate in team meetings, trainings, and planning.
- Provide COVID-19 vaccine and prevention information.
Minimum Qualifications :
High school diploma / GED; strong communication and facilitation skills; community outreach experience; basic computer skills; ability to work independently; ability to lift 25-50 lbs; valid driver's license and reliable transportation; availability for evenings, nights, and weekends.
Preferred Qualifications :
Experience working with vulnerable populations; CHW Certification.

Must be prepared for public health emergency response; must complete required incident management courses; must protect PHI and comply with OSHA requirements.
