Summary
The Community Care Services Nurse Manager position is in the Medical Information Systems (MIS) Division, in the Community Care Program. The incumbent directly supervises the Community Care Coordination Nurses. Provides leadership and support in planning, implementing, assessing, and administering systems to maintain and improve the quality of clinical care coordination. Critical areas of focus include staffing effectiveness, clinical documentation, consult management, and fiscal management.

- Coordinate and design information systems, which support the utilization of the nursing process for the patient data entry, as well as collect, aggregate and analyze data related to various types of health care issues.
- Function in an advanced managerial role whose practice is comprised of complex leadership and administrative components, associated with critical health care and nursing practice issues.
- Accountable for the provision, management, and monitoring of care with Community Care Services.
- Improve Veteran / patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting.
- Responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance.
- Work jointly with the Associate Chief of Staff for Community Care, the Community Care Deputy Program Manager, the Community Care Services Supervisors, and the Community Care Coordination RNs to facilitate and support objectives of Community Care Services.
- Advance knowledge of clinical practice and professional standards of care.

An Associate Degree (ADN) or Diploma in Nursing, with no additional professional nursing required.
Nurse I, Level II
Demonstrates integration of biopsychosocial concepts, cognitive skills and technically competent practice in providing care to clients with basic or complex.
An ADN or Diploma in Nursing AND 1 year of specialized nursing experience equivalent to Nurse I, Level 1 ; OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required.
Nurse I, Level III
Demonstrates proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations.
An ADN or Diploma in Nursing AND 2 years of professional nursing experience in which one year is equivalent to Nurse I, Level 2; OR a BSN and 1 year of professional nursing experience equivalent to the Nurse I, Level 2; OR a Master's degree in nursing (MSN) and no additional professional nursing experience; OR a Master's degree in a
