We are actively looking for Full-Time Peer Recovery Support Specialists in the West Valley Area (including Peoria, Glendale, Buckeye, Goodyear, Surprise) to assist clients as they navigate the criminal justice system or re-entry into the community.

This position will :

• Provide personalized supports to criminal justice involved clients who may benefit from meeting with some-one with lived experience to navigate both natural and professional resources in accordance with goals identified in the client’s service plan.
• Facilitate bimonthly or monthly groups to facilitate skill-building and linkage to community resources.
• Assess and address social determinants of health that impact progress toward goals, including intensive facilitation of access to resources and care coordination (including, but not limited to, facilitating connection to primary care, recovery groups, behavioral health homes, medication assisted treatment, and employment).
• Utilize coaching, mentoring, and role modeling using lived experience according to established PRSS credentialed practices. Interventions may include, but are not limited to, identifying needs and recovery goals, lessening feelings of isolation, increasing practical knowledge to access supports and resources in the community, exploring continued education and / or employment opportunities, partnering with other healthcare practitioners, and overcoming service barriers.

Peer Recovery Support Specialists work primarily in SAGE offices, but may have the opportunity to work remotely; majority of schedule will be in-person meetings with clients and require some travel across Maricopa County to SAGE clinics and sites. Equipment needed to provide services are issued by SAGE.
