SUMMARY OF FUNCTIONS :
The Developmental Disability Professional (DDP) is responsible for coordinating, developing and implementing Individual Support Plans for participants in Frazer Center’s CAG / CAI services. The DDP is at the center of all discussion with the individual, family, guardians, staff and administration. They serve as liaison among all disciplines and team members, and are responsible for direct supervision of CAG / CAI services for Frazer Center participants.
PRIMARY DUTY
- Responsible for overseeing the delivery of waiver services to participants with the focus on overall quality of service delivery and continuous quality improvement

Reports to and is under the direct and primary supervision of the Director of the Adult Day Program. The secondary supervisor is the VP of Programming.
