SUMMARY OF FUNCTIONS :

The Developmental Disability Professional (DDP) is responsible for coordinating, developing and implementing Individual Support Plans for participants in Frazer Center’s CAG / CAI services. The DDP is at the center of all discussion with the individual, family, guardians, staff and administration. They serve as liaison among all disciplines and team members, and are responsible for direct supervision of CAG / CAI services for Frazer Center participants.

PRIMARY DUTY

  • Responsible for overseeing the delivery of waiver services to participants with the focus on overall quality of service delivery and continuous quality improvement

MAJOR DUTIES AND RESPONSIBILITIES :

  • Assuring that the supports provided are within the scope of Frazer Center’s service enrollment and experience
  • Providing, arranging or overseeing the curricula used in staff training directed to service delivery in the context of individual goals and objectives
  • Recommending other needed services / supports or changes to the delivery model using a continuous quality improvement approach
  • Providing consultation to the director, leadership team and staff in ISP implementation strategies that are specific, measurable, achievable, relevant, realistic and time limited in order to meet the needs and goals of the participant.
  • Assesses areas of risk either individual or overall risk to participants served through agency practice, policy or lack of policy and / or procedures. Providing risk mitigation strategies to the Frazer Center leadership team.
  • Reviewing and assuring that functional assessments are in place to support development of the participant’s plan for delivery of all waiver services to include : HRST, SIS, Functional behavioral evaluation, and other assessments;
  • Oversees high intensity services if applicable.
  • Coordinates, plans for, and develops the ISP Goals for assigned participants.
  • Creates outcome measurements and completes quarterly summaries on assigned participant’s progress
  • Conducts face-to-face assessments using a holistic, person-centered approach
  • Communicates effectively and professionally with families, caregivers and other stakeholders.
  • Provides orientation and training to new employees, as well as ongoing training annually and as needed, to ensure appropriate ISP implementation strategies.
  • Accurately maintains records on all participants on their caseload using the Electronic Health Record (Therap)
  • Must provide medical assistance to participants served, such as First Aid, CPR, and / or Heimlich maneuver as trained by the agency.
  • Complete 16 hours of annual training as required by regulation; attend additional training as required by the program / department
  • Provides oversight and guidance to direct support staff and other assigned personnel to ensure services are delivered in accordance with Individual Support Plans (ISPs), agency policies, and regulatory requirements.
  • Participates in an on-call rotation, including availability during designated weekends, to provide consultation, support, and response related to participant needs, staffing issues, or critical incidents, as required by the program.

  • PHYSICAL REQUIREMENTS :

  • Must be able to lift, push, and pull a minimum of 50 pounds
  • Ability to keyboard and sit for extended periods of time
  • Ability to look at a computer screen for extended periods of time
  • ORGANIZATIONAL RELATIONSHIPS :

    Reports to and is under the direct and primary supervision of the Director of the Adult Day Program. The secondary supervisor is the VP of Programming.

    EXPERIENCE AND / OR EDUCATIONAL REQUIREMENTS

  • Bachelor’s degree in social work or in human services field other than social work, or
  • Master’s degree in one of the behavioral or social sciences, psychology, school psychology, counseling, vocational rehabilitation or a related field, or
  • 3 years of experience providing services to participants with intellectual or developmental disabilities
