Here's what you'll be doing

We're looking for an experienced Diabetes Care Specialist to join our team. You'll be responsible for patient experience and success of our practice, including :

• Counsel and educate patients and families appropriately on diabetes care in collaboration with the care team
• Complete nutritional assessments and make appropriate recommendations as a part of the care planning process
• Analyze CGM data and translate to treatment changes for patients
• Communicate with patients through video calls, in person meetings, telephone and electronic messaging
• Create and maintain documentation
• Ensure patient satisfaction by building rapport and maintaining positive relationships with patients and their families
• Other duties as assigned
