A company is looking for a Clinical Guide : Diabetes Educator (CDCES).

Key Responsibilities

Screen members for Diabetes Clinic eligibility and conduct ongoing education on diabetes management

Schedule follow-up appointments and connect members with appropriate resources

Collaborate with team members to develop care plans and optimize medication regimens

Required Qualifications

Active unrestricted Compact Registered Nurse or Registered Dietitian license in specified states

Active Certified Diabetes Care and Education Specialist (CDCES) certification

At least 2 years of experience in clinically managing diabetes

Remote experience managing diabetes is preferred

Experience with diabetes technology (CGMs and insulin pumps) is desirable
