Job description
Job Summary :
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires strong clinical skills and the ability to work independently while maintaining effective communication with patients, families, and healthcare teams.
Duties :
Conduct comprehensive patient assessments to evaluate health status and needs. Develop and implement individualized care plans based on patient assessments. Provide direct patient care, including administering medications, IV insertion, and wound care. Perform physical examinations and monitor vital signs to track patient progress. Educate patients and families on health management, disease prevention, and wellness strategies through health coaching. Collaborate with interdisciplinary teams to coordinate care and ensure optimal patient outcomes. Maintain accurate medical records and documentation in compliance with healthcare regulations. Assist with senior care needs, ensuring a safe and supportive environment for elderly patients.
Skills :
Oasis Experience (Preferred Not Required)Proficient in medical terminology relevant to home health care. Strong patient assessment skills to identify changes in health status. Experience in providing patient care in the home setting Excellent communication skills for effective interactions with patients, families, and healthcare professionals. Compassionate approach towards senior care, demonstrating patience and understanding of their unique needs. Strong organizational skills to manage multiple patients’ needs efficiently.
