Responsibilities :

  • Assess the therapeutic and rehabilitative status of the patient
  • Develop an appropriate treatment plan for each patient; update and modify treatment plan on an ongoing basis.
  • Ensure high-quality patient care
  • Use clinical reasoning in treating and planning implementing and monitoring patient progress
  • Maintain records with accuracy and discretion

    • We set standard base pay ranges for all roles based on function and level. Final offer is determined by multiple factors including skillset work experience and languages and may vary from the amounts listed below.

    The SOC visit pay range for this position is : $115-$145 / visit.

    Qualifications :

  • Complete Start of Care (SOC) assessments for new patients
  • Degree from an accredited Physical Therapy program.
  • Current and unrestricted CA PT license; or ability to apply
  • Excellent Communication Skills
  • Home Health experience preferred; not required

    • Additional Information :

    We are looking for PT SOC coverage in the following areas :   Ventura Oxnard Pasadena Glendale Burbank San Fernando Valley Monterey Park Oak Park Calabasas Agoura Hills San Gabriel Valley West Covina Walnut Montrose Duarte Alhambra and Hacienda Heights.
