Job Description
Job Title : Bilingual Community Health Worker
Location : Austin, TX
Job Type : Full-Time | Exempt
Schedule : Monday–Friday, 8 : 00 AM–5 : 00 PM
Pay : $25-30 / hourly (compensation range is subject to increase for advanced social work degrees)
Benefits : This position is eligible for medical, dental, vision, 401(k), and PTO.
About The Role
Addison Group is hiring an experienced and passionate Community Health Worker (Bilingual – Spanish) on behalf of our valued healthcare partner in Austin, Texas. This is a full-time, onsite opportunity serving an innovative care team focused on improving patient outcomes, health equity, and community engagement for Medicare-eligible adults.
Key Responsibilities
As a Community Health Worker, you will be a vital connection between clients, families, and community resources — advocating for older adults, bridging language and cultural gaps, and helping clients navigate complex care systems. Key responsibilities include :
- Provide community-based support and culturally competent outreach to Spanish-speaking older adults and caregivers.
- Conduct in-person visits in clinical settings, health education, and follow-ups to assess needs and progress.
- Help clients understand and access medical appointments, benefits, and social services.
- Build strong relationships with clients, families, and multidisciplinary care teams.
- Document client interactions and outcomes in electronic health records.
- Connect clients to transportation, food, housing, and other essential community support.
- Act as a trusted advocate to promote engagement, self-sufficiency, and optimal health outcomes.
