Our client in Ventura is looking to fill their RN Case Manager position!

Duties :

  • Develop a care plan that is based on nursing diagnosis.
  • Initial assessment gets completed of patient and family to determine home care needs.
  • Re-evaluates patient nursing needs regularly
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Perform other tasks as necessary.

Qualifications :

  • 2+ years of RN experience
  • Must have a valid driver's license
