Location : Largo, FL

Status : PRN, Per Visit

Shift Hours : 8 : 00 a.m. - 5 : 00 p.m.

Weekend Work : Occasional

On Call : Yes

Home visits in assigned service area : Largo, St. Petersburg, Pinellas Park, and surrounding areas

The Home Health Field LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :

Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse

Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse

Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status

Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

Home Health Field LPN - Requirements :

BLS (Basic Life Support)

LPN (Registered Nurse) License

Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

Required Associate's or Diploma Nursing

Required 1 year Licensed Practical Nurse

Preferred 1 year Home Care

This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be found at .
