The Position :
Catholic Charities of the Archdiocese of St. Louis through our service line of St. Patrick Center, is hiring a Healthcare Navigator to join the Support Services for Veteran Families (SSVF) program. We are looking for candidates with exceptional interpersonal skills for the position of Healthcare Navigator. This role will provide services that include connecting veterans to VA health care benefits or community health care services where veterans are not eligible for VA care. The healthcare navigator will provide assessment, case management and care coordination, health education, interdisciplinary collaboration, coordination, consultation, and administrative duties, as well as closely work with the veteran's primary care provider and members of the veteran's assigned interdisciplinary treatment team. The role requires excellent judgment and must have at least two years of experience in a healthcare or social services area of practice.
