As a Home Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based nursing care for patients with various health conditions.
• Perform assessments, take vitals, administer medication, and document patient progress.
• Educate and support patients and their families in managing illnesses or injuries.
• Work closely with physicians, RNs, and other team members to execute comprehensive care plans.

A few must-haves for Home Health Licensed Practical Nurses:

• Active LPN license in Tennessee or compact licensure and graduation from an accredited nursing program.
• Prior experience in home healthcare preferred but not required.
• Strong clinical skills, communication, and compassion.
• CPR certification and a deep understanding of home health regulatory requirements.
• Trach and Vent Care is required.
