About the Role :

We are seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in Pueblo, CO . You'll work directly with patients to help them improve independence, mobility, and daily function through personalized, evidence-based care.

Compensation & Scheduling :

Full-Time or Part-Time schedules
Competitive Pay : $70 - $85 per routine visit
Start of Care Visits : Up to $120 per SOC
Flexible Schedule : Designed to support a healthy work-life balance while meeting your professional goals

Key Responsibilities :

Conduct comprehensive assessments and develop individualized treatment plans

Deliver skilled therapy focused on ADLs, mobility, home safety, and functional recovery

Educate patients and caregivers on home programs and adaptive strategies

Document patient progress and update care plans as needed

Collaborate with nurses, therapists, and other team members to ensure coordinated care

Requirements

A valid Occupational Therapist license in Colorado

Proven experience in home health or a related field

Excellent communication and interpersonal skills

Proficiency in electronic medical record documentation
