Job Description
We are seeking an experienced Skilled Home Care Registered Nurse with strong infusion and home health experience. The ideal candidate will be responsible for providing high-quality nursing care to patients in their homes, focusing on cardiopulmonary assessment, wound care, and managing complex medical diagnoses.
Responsibilities
- Perform patient assessments and evaluations.
- Administer IV medications and initiate IV therapy.
- Provide wound care management, including dressing changes and wound vac care.
- Manage and care for patients with high-risk conditions such as heart failure, COPD, and post-operative needs.
- Conduct skilled nursing visits, ensuring compliance with Medicare and insurance requirements.
- Document patient care in the electronic health record system.
- Collaborate with interdisciplinary teams, including social services and pharmacy.
