    Job SummaryThe Registered Nurse (Hospice) provides compassionate, patient-centered care to individuals and families facing end-of-life needs. This role ensures quality clinical care, emotional support, and coordination of services in alignment with hospice philosophy and standards.Key ResponsibilitiesDeliver skilled nursing care to hospice patients in homes, facilities, or inpatient settings.Assess patient conditions and develop individualized care plans in collaboration with the interdisciplinary team.Administer medications and treatments as prescribed.Provide education and emotional support to patients and families regarding disease process, symptom management, and end-of-life care.Coordinate care with physicians, social workers, chaplains, and other team members.Document patient assessments, interventions, and outcomes in compliance with regulatory and organizational standards.Participate in on-call rotation to ensure continuous patient support.Required QualificationsCurrent Registered Nurse (RN) license in the state of practice.Must have at least 2 years of recent experience in the specialty within the past 2 years.Strong communication and interpersonal skills.Ability to work independently and manage multiple patients.Commitment to providing compassionate, dignified care.Preferred Qualifications (if any)Prior experience in hospice or palliative care.Familiarity with electronic health record (EHR) systems.Training in pain management and symptom control.Certifications (if any)Basic Life Support (BLS) certification required.Hospice and Palliative Care Nursing Certification (CHPN) preferred.
