A Hospice Licensed Vocational Nurse (LVN) plays a vital role in delivering compassionate, patient-centered care to individuals at the end of life.

As a Hospice Licensed Vocational Nurses (LVN), here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform nursing services which are planned, delegated, and supervised by the registered nurse
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
• Take a holistic approach to care that engages the patient’s mind, body, spirit and family
• Travel throughout the full-service area is required for this role.

A few must-haves for Hospice Licensed Vocational Nurses (LVNs):

• Graduate of an accredited nursing program and active Licensed Vocational Nurse (LVN) license in California
• Minimum of 1 year of home based hospice nursing experience
• CPR certification, valid driver's license, reliable transportation, and valid auto insurance policy
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
