Job description
Long Term Care Pharmacist Are you searching for a meaningful, fast paced, and multifaceted environment where you can be part of the multi-disciplinary team and continue to show your passion for helping patients in need? Apply to our Long-Term Care Pharmacist positions today! As a Long-Term Care Pharmacist, you will work in an exciting, dynamic, and fast paced environment, ensuring the continuity of pharmacy by working with facility leaders and staff to coordinate pharmacy services within long term care facilities, group homes, and medical at home patients. You will have the opportunity to guide and evaluate current services and address/resolve pharmacy related concerns and issues that arise with resident care, medical care, quality of life, and regulatory compliance. The successful Pharmacist will provide outstanding service and ensures that the appropriate medication reaches the patient with both efficiency and safely as well as helping facilities remain compliant in their survey with respect to pharmacy services. Essential job functions:
  • Ensure complete and accurate data entry of orders.
  • Adhere to delivery schedules and routes.
  • Check for completeness and precision on all new and refilled medications orders including cycle fill preparations, IV mixtures, and on demand prescription ordering.
  • Check for content and accuracy of all drug packaging and labeling.
  • Preparation and/or Supervision of Sterile IV drug products or Sterile products.
  • Preparation and/or Supervision of Non-Sterile Drug Products.
  • Assist in maintaining a perpetual inventory in the pharmacy.
  • Knowledge of DEA and state law with regards to controlled substances and dangerous drugs as well as the feasibility of dispensing a medication based on patient history.
  • Ensure facilities remain compliant and aligned with federal and state regulations.
  • Complete clinical reviews and/or medication management therapy reviews.
  • Use existing customer relationships to promote and drive growth as well as helping to develop new business lines.
  • Conduct Executive Reviews and other customer meetings as required (immunization clinics, administrators).
  • Educate facility staff including Director of Nurses, Medical Director, and Administrators when new programs are rolled out as well as educate facility staff on topics the Facility Leadership may request.
  • Adhere to Federal and State Board of Pharmacy Regulations as well as USP 795, 797, and 800 Regulations.
  • Answering questions from family members or patients regarding their medication management including counseling on compounded medications.
  • Notify and follow up with customers when out of stock or delays in dispensing may occur.
  • Participation in the Quality Assurance Program.
  • Requirement to take On-Call.
  • Other duties as assigned as needed based on business objectives.

Education and Certifications Required:
  • Bachelors Degree (Pharmacy) or PharmD. Required.
  • State of Texas Pharmacist License required in good standing without restrictions.
  • Sterile Compounding Certification required and up to date.
  • Immunization Certification and CPR required and up to date.

Skills and Qualifications:
  • 2 + years of Long-Term Care Experience.
  • Advanced Computer skills; pharmacy operations experience preferred.
  • Ability to work independently as well as motivate the team to delivery excellent service to meet deadlines.
  • Highly skilled at solving problems with minimal direction.
  • Attention to Detail and Accuracy.
  • Ability to work in a fast-paced changing environment while working on multiple tasks simultaneously.
  • Ability to give feedback in the moment and lead the support staff to provide excellent patient care.

Work Environment:
  • Ability to work Flexible Hours including nights, weekends, holidays and on-call.
  • Full and Part Time Positions Available.
