Job description

UPMC Presbyterian, a premier adult tertiary care and academic medical center located in the heart of Oakland, is seeking a highly motivated Clinical Pharmacist to join our Transplant Program. As a cornerstone of one of the nation’s leading health systems, UPMC Presbyterian offers a collaborative, academically rich environment focused on delivering exceptional patient care.

As a pioneer in the field of solid-organ transplantation, our program consistently accepts and treats some of the most difficult and complex cases. As a Clinical Pharmacist, you will be a vital member of the multidisciplinary care team, delivering comprehensive medication management across a broad range of transplant patients. Your role will include dosing and managing immunosuppression therapy, optimizing pharmacotherapy, supporting transitions of care, contributing to antimicrobial stewardship efforts, and ensuring the safe and effective use of medications.

This position offers the opportunity to engage in clinical teaching, quality improvement initiatives, and interprofessional collaboration, all within a culture that values innovation, continuous learning, and clinical excellence.

Responsibilities:

Deliver direct patient care through real-time medication management within assigned transplant services (liver, heart, lung, kidney), with potential support of additional care areas and teams such transplant flex/ambulatory infusion center, and chronic renal team

  • Participate in transplant pharmacy consult services which include calcineurin inhibitor, warfarin, and thymoglobulin

  • Facilitate acute interventions including medication procurement, preparation, and therapeutic monitoring

  • Provide timely drug information and evidence-based recommendations

  • Participate in transplant-focused research, quality improvement, and practice advancement initiatives

  • Precept pharmacy students and residents that include PGY1 and PGY2; contribute to ongoing education of clinical staff

  • Lead and manage assigned patient service lines, clinical areas, and therapeutic programs.

  • Facilitate pharmaceutical care services and medication utilization systems to ensure alignment with patient needs, evidence-based practices, and regulatory standards.

  • Design and implement stewardship, restriction, and surveillance programs for medication use.

  • Monitor and evaluate pharmacy programs for operational, quality, and financial efficiency, benchmark against best practices.

  • Develop and oversee policies and procedures for drug purchasing, usage, distribution, and control.

  • Provide direct patient care and clinical pharmacy services, including decentralized and service-based programs.

  • Collaborate with multidisciplinary teams to deliver patient-centered care, manage high-risk patients, and participate in medical emergencies.

  • Ensure safe, accurate, and timely medication therapy through monitoring, reconciliation, counseling, and prevention of drug-related problems.

  • Oversee pharmacy operations and technician activities, including specialty medication procurement and dispensing.

  • Advance education and practice by precepting students/residents, developing training programs, and leading quality improvement initiatives.

  • Graduate of an ACPE accredited College of Pharmacy
  • B.S. degree in Pharmacy Doctor of Pharmacy (PharmD) degree; PharmD preferred.
  • Current licensure as a Pharmacist in practicing state
  • Completion of ASHP accredited PGY-1 Pharmacy Residency or 3 years clinical pharmacy experience
  • Knowledge and expertise in advanced pharmacy practice and service, comprehensive medication management, and evidence-based medicine in specialized area appropriate for clinical assignments
  • BPS Board Certification preferred.
  • ASHP accredited PGY2 in Transplant, Transplant Fellowship, or 3 years of experience in a clinical transplant position preferred.
