Hospice Social Worker (LMSW/LBSW) - PRN
Location: Beaumont and the surrounding area


Looking for meaningful work where you can truly support patients and families?Concho Hearts Hospice is seeking a compassionate PRN Medical Social Worker (LMSW or LBSW) to support patients and families in the Beaumont area.
Hospice social work is about more than assessments; it’s about helping families navigate one of life’s most difficult journeys with support, guidance, and compassion. Our Social Workers play a vital role in providing emotional support, connecting families with resources, and helping patients maintain dignity and comfort.
If you’re looking for purpose-driven work with a supportive interdisciplinary team, we’d love to meet you.
What You’ll Do:
  • Assess the psychosocial needs of hospice patients and their families.
  • Ability to provide compassionate emotional support to patients and families during end-of-life care.
  • Assist families in identifying community resources and financial or social support services.
  • Collaborate with the RN Case Manager and interdisciplinary team to develop individualized care plans.
  • Participate in interdisciplinary team meetings (IDT) to support patient care.
  • Assist with bereavement planning and coordinate with the Bereavement Coordinator as needed.
  • Document assessments and interventions in the EMR system.

What We’re Looking For:
  • Licensed Social Worker in Texas (LMSW or LBSW).
  • Master’s or Bachelor’s degree in Social Work from an accredited program.
  • Minimum 1 year of social work experience in healthcare, hospice, or long-term care.
  • Hospice experience preferred but not required.
  • Strong communication and counseling skills.
  • Ability to work independently while collaborating with a care team.
  • Reliable transportation and willingness to travel locally.
