What You Will Do to Change Lives
Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Full-Time | DAYS | Shifts: : AM - : PM | Days: Monday - Friday
Expected starting wage range is $ - $ Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Telecare’s community program in Central Orange County, Homeless General Population will focus on addressing the complex housing needs of up to members, many of whom have long histories of homelessness and serious mental illness. The program will emphasize enhanced engagement and outreach efforts along with intensive case management, Vocational/Educational/Employment support and robust housing strategy services.
