General Summary

The HIA Health Care Consultant is responsible for developing, nurturing, and expanding relationships with referral sources across our service area. This role advocates for appropriate HIA Hospice and HIA House Calls referrals through education, outreach, and consultative relationship-building. The Consultant serves as a bridge between healthcare providers, community organizations, and HIA Health's care teams - ensuring that patients and families receive timely access to compassionate, high-quality care.

Ideal candidates may come from a healthcare background (LPN, CNA, MA, SW, etc.) or from a consultative marketing/sales environment but must share a passion for service, advocacy, and community health.

Essential Functions:

• Grow company census and expand territory through responsible and mission-aligned marketing practices.
• Promote HIA Health through community engagement, professional presentations, and relationship-based outreach.
• Educate referral partners on HIA Hospice and HIA House Calls services, Medicare/Medicaid/insurance benefits, and eligibility guidelines.
• Develop and execute a territory marketing plan that drives appropriate referrals.
• Analyze referral patterns and adjust strategies to improve results.
• Identify, cultivate, and prioritize key referral relationships (physicians, discharge planners, senior organizations, facilities, etc.).
• Collaboration with the clinical and admissions teams by facilitating smooth communication and coordinated transitions of care.
• Maintain detailed CRM documentation (Trella) and track activity, trends, and referral outcomes.
• Participate in community education, health fairs, facility events, and special projects representing HIA Health.
• Collaborate with internal departments - admissions, clinical, education, and marketing - to ensure consistent and excellent service delivery.
• Uphold HIPAA and confidentiality standards.
• Assist with obtaining required clinical documentation, including physician signatures.
• Represent HIA Health with professionalism, warmth, and HEART values in all interactions.
• Perform additional duties as assigned.

Pay Range: Starting at $25.76/hour (based on experience)

Education, Certificates and Experience:

• Bachelor's degree in marketing or business administration.
• Healthcare professional or individual with referral development, Marketing or sales background preferably in the healthcare environment, with a minimum of two years of experience. Preference for those with prior healthcare experience as an LPN, CNA, MA, SW, etc.
• Strong verbal and written communication skills.
• Self-starter with ability to create leads.
• Able to maintain an organized approach to territory management and work with limited supervision.
• Able to support and promote the Hospice philosophy to referral sources in the medical industry.
• Valid driver's license and reliable transportation.
• Formal sales training highly preferred.
• Hospice/LTC experience, preferred.
• Current driver's license and auto insurance required
• Bachelor's degree in marketing, business, healthcare administration, or related field preferred.
• OR clinical certification/experience (LPN, CNA, MA, SW, etc.).
• Minimum two years of experience in healthcare, referral development, sales, community outreach, or a related field.
• Hospice, home care, or long-term care experience preferred.
• Formal sales training strongly preferred.
