Dialysis Clinical Liaison

Lexington Renal is a state-of-the-art, in-facility dialysis suite located within Roosevelt Rehabilitation & Healthcare Center in Northeast Philadelphia. Designed to meet the growing regional demand for high-quality dialysis access, Lexington Renal offers both inpatient and outpatient dialysis services and can accommodate up to 108 total patients.

We are seeking a dynamic, relationship-driven Dialysis Liaison to serve as the key connector among our program, hospital partners, and nephrology networks. This individual will be responsible for driving census growth, developing strong physician and hospital partnerships, and ensuring seamless coordination of care for patients requiring dialysis services.

Responsibilities:
• Develop and grow referral relationships with hospital case managers, discharge planners, nephrologists, physician practices, and dialysis centers to increase awareness and utilization of Lexington Renal.
• Educate referral sources and community partners on Lexington Renal’s capabilities, services, and patient outcomes through in-services, tours, and outreach presentations.
• Collaborate closely with nephrology groups and hospital-based renal programs to create efficient transition pathways for both inpatient and outpatient dialysis needs.
• Track and report referrals daily; maintain accurate logs and communicate outcomes and updates to department heads and leadership.
• Serve as a resource and liaison for hospital partners and families to ensure timely admissions, clear communication, and continuity of care.
• Participate in marketing initiatives and community engagement efforts to strengthen brand visibility and patient access to care.
• Represent Lexington Renal at professional events, health fairs, and industry conferences to promote program awareness and build lasting professional relationships.
• Uphold the mission, vision, and values of Lexington Renal and Roosevelt Rehabilitation & Healthcare Center through professionalism, integrity, and commitment to patient-centered care.

Qualifications:
• Valid driver’s license and ability to travel to hospitals, physician offices, and community settings regularly.
• RN, LPN, or healthcare background strongly preferred.
• Associate’s degree required; Bachelor’s degree preferred.
• Proven experience in hospital liaison, dialysis, or post-acute care marketing is highly desirable.
• Excellent interpersonal, communication, and relationship-building skills.
• Highly organized, self-motivated, and goal-oriented with the ability to manage multiple priorities effectively.
