[job_card.job_description]Graduate from an accredited School of Nursing. Minimum of two years of experience in Medical-Surgical Nursing, Community Health, Hospice, or Oncology nursing is preferred. Valid New York State RN licensure; a valid NYS driver's license is preferred. Willingness to sit for national board certification (CHPN) within one year of hire. Proficiency in Word, Excel, and Windows applications.

We deliver a comprehensive range of services, whether in community settings or facilities, designed to support patients and their families during challenging times.
As an On-Call Registered Nurse, you will be vital in providing on-site patient visits, both scheduled and unscheduled, to address urgent physical and psycho-social concerns during after-hours shifts. Your role will involve :
- Coordinating with Call Center Triage and the On-Call Manager to respond effectively to triage visit requests.
- Communicating promptly with the on-call team, including the Hospice physician, to meet urgent patient needs and establish safe care plans.
- Implementing care plans, while educating patients and primary caregivers on medication management, patient safety, and symptom management.
- Providing emotional support and guidance to families during patient deaths, as directed.
- Reporting patient-related visit details, changes to care plans, and follow-up needs to the primary Hospice team and triage.
