We are seeking a highly organized and motivated individual to join our team as a Facilitated Telehealth Coordinator in the HealthCare industry. As a Facilitated Telehealth Coordinator, you will play a crucial role in coordinating and supporting telehealth services for our patients. This position offers an exciting opportunity to contribute to the advancement of healthcare technology and improve access to care for our patients
WHAT IS YOUR SUPERPOWER?
- Facilitate communication between providers and their patients including assisting patients use of technology to conduct virtual visits with other providers on the team
- Assess environment regarding safety concerns or potential hazards
- Communicate with the patient care team regarding any patient questions and needs such as medications, Community Based Organizations.
- Coordinate telehealth services and ensure seamless delivery of care to patients in various settings : home, clinic, living communities (ALFs, etc.)
- Provide education and support to patients and caregivers on how to use telehealth technology effectively
- Maintain accurate and up-to-date records of telehealth appointments and patient data
- Collaborate with IT and technical support teams to resolve any telehealth system issues
- Conduct training sessions to educate healthcare staff on telehealth best practices and workflows
- Collaborate with cross-functional teams to identify technology needs and opportunities for improvement
- Troubleshoot and resolve technology issues in a timely manner to minimize downtime.
