About Blize Healthcare:Blize Healthcare is dedicated to delivering high-quality, compassionate care to patients in the comfort of their homes. We are seeking a skilled and patient-focused LVN (IV Certified) to join our growing home health team.

Position Overview:As an LVN, you will provide skilled nursing care in a home health setting, working under the supervision of a Registered Nurse or physician. This role requires strong clinical skills, independence, and a compassionate approach to patient care.

Key Responsibilities:
  • Perform comprehensive patient assessments and monitor ongoing progress.
  • Develop, implement, and follow individualized care plans.
  • Administer medications, including injections and IV therapy.
  • Provide wound care, including dressing changes and monitoring for infections.
  • Educate patients and their families about their conditions, medications, and care plans.
  • Accurately document patient care, assessments, and progress notes.
  • Observe and report changes in patient condition to supervising clinicians.
  • Collaborate with interdisciplinary teams to ensure quality care.

Qualifications:
  • Active LVN license (California).
  • IV Certification (required).
  • Valid BLS Certification.
  • Minimum 1 year of nursing experience.
  • Home Health or Hospice experience preferred.
  • Strong skills in medication administration, wound care, and patient assessment.
  • Excellent communication and interpersonal skills.
  • Ability to work independently in a home health environment.
  • Ability to lift up to 50 lbs as needed.
  • Valid Driver’s License and reliable transportation.
