• Assesses an individual’s nursing and medical needs
• Develop and individual’s plan of care
• Obtain physician’s orders for medication and treatment to be administered
• Determine whether self-administered medications have been appropriately taken, applied, or used
• Enters, dates, and signs monthly progress notes on medical care provided
• Administer medication and treatment
• Provides health education and
• Maintain medical records
• Must be on site 8 hour per day Monday – Friday
• If an LVN is employed as the facility nurse, then an RN consultant must provide consultation at the facility at least four hours per week. The RN consultant must document the consultation provided, and must be provided when individuals are present in the facility
