Nurse Manager I (RN) – Community Health Clinic
Position Overview:
Under the supervision of the Chief Nursing Officer (CNO)/Deputy Nursing Officer (DNO) and in collaboration with the Site Medical Director, the Nurse Manager I (NM I) is responsible for delivering culturally competent care to underserved adult populations across Los Angeles County.
This role provides clinical leadership and operational oversight to multidisciplinary staff, ensuring the delivery of high-quality, cost-effective patient care. The NM I plays a key role in care coordination, workflow optimization, and implementation of Patient-Centered Medical Home (PCMH) initiatives to improve patient outcomes and clinic efficiency.
Key Responsibilities
Leadership & Team Management
- Provide direct supervision and leadership to RNs, LVNs, Medical Assistants, HIV/STD Counselors, Phlebotomists, and other clinical staff.
- Foster a collaborative, patient-centered team environment focused on quality care delivery.
- Support staff performance, training, and adherence to clinical protocols and regulatory standards.
Clinical Care & Patient Services
- Provide clinical support to providers to ensure timely, high-quality patient care before and after adult primary care visits.
- Deliver care in accordance with established clinical guidelines and best practices.
- Administer adult immunizations in compliance with clinical protocols.
- Conduct patient triage for walk-ins and urgent clinical concerns.
- Provide chronic disease management (CDM) services, including care plan development and coordination with providers.
- Deliver and document patient education and preventive health counseling.
- Delegate appropriate tasks, including CDM support, to LVN staff.
Care Coordination & Clinic Flow
- Coordinate with clinical and support staff to ensure timely specialty referrals and continuity of care.
- Partner with front office teams to support patient scheduling adjustments and access to care.
- Lead or participate in daily huddles to align on patient care priorities and clinic operations.
- Ensure efficient patient flow and resource utilization to minimize delays in care delivery.
Quality Improvement & Operations
- Participate in and support quality improvement initiatives in collaboration with the Medical Director.
- Analyze operational and clinical data, including patient outcomes, provider productivity, and financial performance metrics.
- Partner with leadership to enhance clinic efficiency, patient experience, and cost management.
- Ensure cost-effective supply management and resource utilization.
- Identify and implement opportunities to improve workflows, increase staff engagement, and optimize care delivery.
Required Qualifications
- Active Registered Nurse (RN) license
- Strong experience in patient triage, clinical care, and documentation review
- Proficiency with EMR systems
- Working knowledge of OSHA and HIPAA regulations
- Proven ability to lead clinical teams in a fast-paced healthcare environment

This is a Contract to Hire position based out of West Hollywood, CA.
Pay and BenefitsThe pay range for this position is $110000.00 - $145000.00/hr.
