Job description

The Nurse Practitioner is responsible for providing high-quality, patient-centered primary and/or specialty care through the assessment, diagnosis, and treatment of patients across the lifespan. They will also play a key role in patient education, health promotion, disease prevention, and the accurate documentation of care in accordance with clinical, regulatory, and organizational standards.

Reports to: Chief Medical Officer

Responsibilities:

  • Serve as a role model and advocate in the delivery of compassionate, comprehensive care to patients and their families.

  • Utilize expert knowledge, skills, and abilities to strengthen clinical practice in their specialty throughout the organization.

  • Provide expert nursing care to selected patient population.

  • Evaluate patient outcomes and revise plans of care accordingly.

  • Participate in the development and evaluation of patient and family education materials.

  • Promote the professional development of all nurses through specialty training and education.

  • Demonstrate the knowledge, skills, and ability needed to ensure care appropriate to the age and needs of the population served.

  • Facilitate research interpretation and clinical studies to advance clinical practice.

  • Gather pertinent information systematically and skillfully from all sources: history, diagnostic tests, comprehensive physical examination, and psychosocial assessment.

  • Provide total comprehensive care (supervised by a Physician) to selected clients who satisfy the criteria as outlined in the description of service. Responsible for ante-partum, intra-partum, and inter-conceptual care of these selected clients.

  • Differentiate normal and abnormal variations for all body systems.

  • Assess client and family adaptation coping skills and the need for crisis or other intervention.

  • Accurately interpret diagnostic test.

  • Communicate with the medical staff of any abnormalities or complications. This consultation may result in co-management or referral of client to medical management.

  • Develop problem lists with associated differential diagnosis.

  • Identify educational needs of the client and family and assist with teaching.

  • Incorporate assessment finding into a plan to direct client care.

  • Formulate a plan of care in collaboration with the client, family, physician, nursing staff and other members of the health care team which incorporates health care maintenance, teaching, and follow-up care.

  • Appropriately prioritize client care.

  • Plan and implement appropriate pharmacological therapies.

  • Accurately and appropriately perform routine diagnostic and therapeutic techniques according to established protocols and standardized procedures.

  • Initiate referrals based on client and family need.

  • Seek appropriate consultation dependent upon client health care needs.

  • Accurately collect, document, and communicate patient history, examination findings, clinical assessment, and plan of care in the electronic medical record and with the interdisciplinary care team.

  • Identify and promote cost effective, quality client care.

  • Establish and maintain a collaborative relationship with health care colleagues.

  • Demonstrate effective use of verbal, non-verbal, and written communication skills using strategies that promote problem resolution.

Experience:

  • Five to seven years of clinical experience, at least three in the specialty area of care preferred.

  • Ability to maintain a high degree of accuracy in carrying out work duties.

Education:

  • Graduate of an accredited school of nursing.

  • Master’s degree in nursing required.

  • Current RN license in the State of Illinois required.

  • Current APN license in the State of Illinois required.

  • Certification as an advanced practice nurse by the appropriate certifying organization required.
