The UW Medicine Harborview Comprehensive Stroke Center is seeking an experienced, collaborative, and patient-centered Stroke Nurse (RN) Navigator to join our team.
WORK SCHEDULE
Full-time, 100% FTE (40 hoursper week)
Monday-Friday(0800-1630)
On-site at Harborview Medical Center
DEPARTMENT DESCRIPTION
The Comprehensive Stroke Center at Harborview Medical Center is a Joint Commission-certified and Washington State-designated Level I Comprehensive Stroke Center , providing high-acuity, evidence-based care to stroke patients across the WWAMI region (Washington, Wyoming, Alaska, Montana, and Idaho).
Our multidisciplinary team is dedicated to improving patient outcomes and access to care through cutting-edge interventions, regional partnerships, telestroke outreach, and robust post-discharge coordination.
Our multidisciplinary team of the stroke center is a nationally and internationally recognized leader in stroke care, innovation, and health equity-delivering coordinated, person-centered services across the care continuum through inpatient, outpatient, and Telestroke partnerships.
The Stroke RN Navigator plays a vital role in patient and family education, complex care coordination, and clinical collaboration throughout the continuum of stroke care-from admission through discharge and outpatient follow-up.
POSITION HIGHLIGHTS
The Stroke RN Navigator plays a critical role within the Comprehensive Stroke Center at Harborview. This role reports directly to the Stroke Program Manager / Director and works in close collaboration with :
Neurologists, neurosurgeons, hospitalists, and stroke fellows / residents
Care management, social work, and therapy services
Outpatient stroke and neurology clinics
Telestrokeand WWAMI regional referral partnersand local providers
PRIMARY RESPONSIBILITIES
Patient Education & Family Support
Deliver individualized, in-person stroke education at the bedside
Reinforce understanding of diagnosis, medications, stroke warning signs, and lifestyle modifications
Support family engagement and caregiver readiness throughout the hospitalization
Inpatient Navigation & Care Coordination
Identifypatients through daily census and discharge reports
Conduct chart reviews for patients admitted within the last 12-72 hours
Attend andparticipatein interdisciplinary discharge rounds
Collaborate in person with medical teams, care management, and therapy tofacilitatesafe,timelydischarges
Coordinate services with the Comprehensive Care Nurses (CCN), Care Assurance Team (CAT), and external partners as needed
Post-Discharge Follow-Up & Continuity of Care
Perform outreach calls to discharged patients to ensure successful transitions
Coordinate follow-up appointments with primary care and specialty providers
Facilitate post-discharge imaging, lab work, or therapy services at HMC or local facilities
Assistwith outpatient stroke clinic referrals and care handoffs
Program Support & Quality Improvement
Attendandparticipateinstroke operations and performance improvementefforts as directed
Support chart reviews, data collection, and quality audits
Support andparticipateinmonthly Stroke Support Group, UW Stroke Club,for survivors and families
Helpidentifygaps in care, patient safety concerns, or workflow barriers and escalate to stroke leadership
REQUIRED QUALIFICATIONS
Bachelor of Science in Nursing (BSN)
Current license to practice as a Registered Nurse (RN) in the State of Washington
Minimum 3 years of RN experience , ideally in neurology,neurosurgery,critical care, acute care, or care coordinationworking closely with stroke neuro patient population
Strong communicationskills, both verbal and written, and ability to engage with multidisciplinary providerswithout constant direct supervision
Comfort with EpicEHR system,MS Office software, and other care coordination tools
