Position: Admissions Coordinator
Schedule: Full-Time
Salary: $26–$33 per hour
Charming Lakes Rehabilitation is seeking a motivated, detail-oriented, and compassionate Admissions Coordinator to join our team. In this vital role, you will be the first point of contact for residents and their families—ensuring a seamless admissions experience while building strong relationships with referral sources and the surrounding community.
Identify and monitor operational areas impacting admissions; report findings and assist with process improvements
Support marketing and outreach efforts by attending community events and developing referral relationships
Educate families and guests on facility policies, visitor guidelines, and resident care procedures
Coordinate admissions with hospitals and referral sources to ensure smooth and timely transitions
Build and maintain strong relationships with hospitals, physicians, and community partners
Verify financial and insurance information prior to admission and communicate details to the interdisciplinary team
Maintain up-to-date knowledge of Medicare, Medicaid, and Managed Care processes
Track, analyze, and report admissions data, trends, and performance metrics
Minimum of 2 years of admissions experience in a Skilled Nursing Facility
Strong understanding of Medicare, Medicaid, and insurance processes (including PDPM, HMO guidelines, and RUGS II & III)
Excellent communication, organizational, and interpersonal skills
