NorTek Medical Staffing is seeking a compassionate and skilled Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join a growing home health team in Los Angeles County. This role focuses on delivering high-quality, patient-centered rehabilitative care in the comfort of patients’ homes, supporting healing, independence, and long-term mobility.

Position Overview

In this role, you will evaluate patients, develop individualized plans of care, and provide hands-on therapy services while collaborating closely with physicians and an interdisciplinary care team to achieve optimal patient outcomes.

Key Responsibilities

Conduct comprehensive initial evaluations and develop individualized Plans of Care

Provide physical therapy services in accordance with physician orders and agency policies

Educate patients and caregivers on mobility, strength, safety, and functional independence

Reassess patients and update treatment plans as conditions change or after hospitalization

Document all patient care accurately and in a timely manner per state and agency standards

Communicate patient progress and concerns with physicians, nurses, and office staff

Participate in interdisciplinary case conferences and quality improvement initiatives

Supervise and mentor PTAs as applicable

Promote patient safety, independence, and quality of life

Maintain professional growth through ongoing education and training

Required Qualifications

Active California Physical Therapist license

Valid driver’s license with reliable vehicle and current auto insurance

Ability to travel to patient homes throughout Los Angeles County

Strong interpersonal, communication, and organizational skills

Physical ability to lift, transfer, and assist patients

Preferred Qualifications

Experience in home health, hospital, or outpatient rehabilitation

Bilingual communication skills

Familiarity with EMR systems

Required Credentials (Must Be Current)

California Physical Therapist License

Driver’s License & Auto Insurance

TB Test or Chest X-Ray

Physical Exam

CPR Certification
