Job Summary : We are looking for a dedicated and empathetic Physical Therapist to join our team, playing a crucial role in helping patients regain their strength, mobility, and confidence in our outpatient clinic. Collaborating with a supportive interdisciplinary team, you will create personalized, evidence-based treatment plans aimed at addressing each patient's unique needs and rehabilitation goals.

Responsibilities :

• Conduct thorough initial and ongoing assessments to evaluate patients' mobility, strength, balance, and functional limitations.
• Develop and implement tailored treatment plans based on evidence and the specific goals of each patient.
• Utilize a variety of therapeutic techniques, including exercises, manual therapy, and gait training, to enhance function, restore mobility, and alleviate pain.
• Educate patients, family members, and caregivers on treatment plans and the effective use of assistive devices, alongside suggesting in-home exercises for continued recovery.
• Provide clinical supervision to Physical Therapist Assistants (PTAs) to ensure care aligns with established plans and industry standards.
• Maintain accurate and comprehensive documentation of patient progress, evaluations, and treatments in the electronic health record (EHR) system.
• Collaborate closely with physicians, nurses, occupational therapists, and social workers to coordinate care and ensure a seamless transition for patients discharging home.
• Engage actively in care planning meetings and interdisciplinary team discussions.
• Adhere to all company, state, and federal regulations regarding patient care and documentation.
