A company is looking for an Outreach Coordinator to enhance member health outcomes through community collaboration and member engagement.

Key Responsibilities

Identify and collaborate with community organizations to improve member health outcomes

Make outbound calls to assist members in obtaining services and improving health outcomes

Develop educational materials and maintain member incentive programs to support health initiatives

Required Qualifications

3+ years of recent outreach experience with providers or members, or government program managed care experience

Experience with Microsoft Office products (Word, Excel, Project, PowerPoint, Outlook)

High School Diploma or GED (required)
