As a Bridge Care Coordinator, you'll be a steady, compassionate presence in patients' lives, helping them and their families navigate their healthcare journey with dignity, education, and understanding.This is a relationship-driven role for someone who cares deeply about older adults, works well independently, and builds trust naturally with patients, caregivers, and healthcare partners alike.Your key responsibilities will include:Build lasting relationships through non-clinical visits in homes, assisted living communities, and skilled nursing facilities.Offer emotional support and companionship while honoring each patient's goals and independence.Educate patients and families on disease progression, community resources, and advanced care planning.Serve as a trusted point of contact, recognizing changes in patient needs and sharing them with the clinical team.Collaborate with physicians, nurses, social workers, and facility partners to keep care coordinated and patient-centered.Maintain accurate, timely documentation of every interaction.What Makes You a Great FitYou're compassionate, organized, and self-motivated.
