[job_card.job_description]Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness Monitor the patient's progress and assessment results to communicate clinical and technical information with the physician and other members of healthcare team. We are seeking a highly skilled and motivated Home Infusion Pharmacist to join our team. As a Home Infusion Pharmacist, you will be responsible for reviewing charts and lab results for an assigned caseload of patients receiving specialized, complex infusion therapy. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the highest quality of care. Report to : Pharmacy Supervisor
Job Type : Full-Time, 40 hours per week, Mon-Fri 8 : 00am-5 : 30pm CST
Location : Lenexa, KS
Requirements
- B.S. or Doctor of Pharmacy degree is required or Registered pharmacist license if foreign graduate
- Active Kansas pharmacist license in good standing
- Availability Monday through Friday and participation in the on-call rotation. On-call requirements are approximately one weekend / in 3 weeks. The hours of operation are 8 : 00am-5 : 30pm CST
