Job Description

About us :

Arizona Home Care is a closed-door home infusion and homecare facility that was first established in 1995. Our service includes both acute and non-acute patient populations by bringing the hospital room to the patient's home. We work with all the major hospitals and providers in the valley including but not limited to : The Mayo, HonorHealth, Banner, Dignity, Valleywise, St. Joes, Hospice of the Valley, and Abrazo.

About the Role :

As a Pharmacist at Arizona Home Care, you will play a vital role in the care of our patients through :

  • Processing prescriptions and med refills
  • Creating accurate prescriptions
  • Communicate med change and cost changes to patients
  • Addressing side effects and abnormal labs with patients and provider
  • Supporting and leading a team of technicians
  • Prioritizing deliveries and backend drug verification based on delivery expectations

Qualifications :

  • Licensed Pharmacist in the state of Arizona
  • Friendly and confident on the phone
  • Ability to spend long periods of time on your feet

    • Nice to Have :

  • Experienced with our EMR : CareTend
  • Experienced in home infusion
  • Experienced in sterile compounding

    • Schedule :

  • 10 : 30 AM to 7 : 00 PM Monday through Friday

    • Requirements

    1. Bachelor of Science in Pharmacy with a current Arizona pharmacist license.

    2. Master of Science or Doctor in Pharmacy desirable.

    3. Continues completion of appropriate CEU’s for licensure.

    4. Requires experience and a strong knowledge base in intravenous admixture services and intravenous therapy.
