The RN works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family centered care. The RN systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The RN utilizes evidence to drive clinical decisions and practice related to individual and population health. The RN establishes goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. The RN considers factors related to safety, effectiveness and fiscal accountability in planning and delivering care. The RN participates in continuous quality improvement initiatives and assumes responsibility for their professional development.
ESSENTIAL FUNCTIONS
- Acts as resource to medical support staff. Collaborates with medical staff in carrying out delegated patient care activities.
- Seeks educational opportunities to increase clinical competence, personal and professional growth through in-house education material, conferences, and community resources.
- Promote a positive working environment that represents cooperation and collaboration between programs, services and co-workers.
- Must be willing to use technology to meet competency and performance measurements and evaluations.
- Actively participate in team and company meetings, in-services, and other events.
- Maintain strict confidentiality according to HIPPA regulations.
- Demonstrate tact, courtesy and respect in all interactions with patients, visitors, and staff as well as outside agencies.
- Participate in quality efforts to promote appropriate care and services.
EDUCATION AND EXPERIENCE:
- Graduate of an accredited nursing program.
- Currently licensed in the State of Wisconsin.
- Two years of general nursing in a clinic or hospital setting, and previous experience with patients of all age groups.
- Experience within a community health center and with EPIC preferred.
