Psychiatric Nurse Practitioner – Long-Term Care (Adults 18+)

Our psychiatric nurse practitioners partner closely with each facility’s long-term care healthcare team to deliver individualized, high-quality mental health care for residents. • Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.

Requirements for this position include:

• Graduate of an accredited Nurse Practitioner program.
• PMHNP license required.
• Experience in psychiatric, mental, and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.
• Outpatient and tele-psychiatry experience a plus.
• Willingness to travel within assigned service area.
