Psychiatric Nurse Practitioner

MindCare Solutions is seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our dynamic team on a full-time, part-time or PRN basis . As a leading prover of mental health services, we are committed to providing exceptional care and support to our geriatric patients and residents who are facing mental health challenges in the LTC community. Compensation : Productivity based- earning $75 - $110 per hour

Location : Pittsburgh, PA

Hours : Full-time, PRN or Part-time

As a PMHNP with MindCare Solutions, you will collaborate with interdisciplinary teams to deliver high-quality, value-based psychiatric care across a variety of clinical settings. You will focus on proactive, evidence-based treatment strategies that improve the health and well-being of individuals while strengthening the capacity of partner organizations. This dynamic role emphasizes adaptability, innovation, and a patient-centered approach to behavioral health care. Reporting to Chief Medical Officer.

Essential Duties and Responsibilities

• Conduct Comprehensive Psychiatric Evaluations : Perform thorough psychiatric assessments to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care.
• Ongoing Medication Management : Develop and adjust treatment plans, including medication management, based on patient progress and input from the interdisciplinary team.
• Collaborative Care Planning : Partner with case managers and interdisciplinary teams to create and refine care plans, ensuring a seamless, integrated approach to behavioral health management.
• Team Engagement : Participate in regular team meetings to evaluate new referrals, review active cases, and adjust care plans based on patient progress and organizational goals.
• Engagement in Quality Programs : Contribute to quality initiatives such as performance improvement programs, medication reviews, and outcome optimization strategies.
• Travel : Travel up to 50% of the time to meet with healthcare partners, attend industry events, and provide consistent support across clinical settings.
