[job_card.job_description]Conducts initial comprehensive psychiatric evaluations and monthly psychiatric assessments in the community; Prescribes medications through e-prescribing function of agency's electronic health record, educates consumers about their medications and side effects, and monitors consumers' response to prescribed medications; Coordinates consumers' medication management needs with the YA ACT nurse, including ordering blood work and / or toxicology testing when indicated, managing prior authorizations for medications, administering injectable medications when needed; Completes and signs assessments, progress notes, incident reports, and other required documentations in accordance to the New York State Office of Mental Health (OMH) regulations, the New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards and regulations; Coordinates care with family and other community service providers or collaterals including, but not limited to : hospitals, housing providers, shelter staff, criminal justice personnel, Assisted Outpatient Treatment (AOT), and primary health care providers; Participates in team meetings and clinical meetings to collaborate and discuss clinical issues and clients' needs with other team members; Uses motivational interviewing, developmentally and culturally appropriate, recovery oriented, and trauma-informed approaches when delivering services; Any other duties are required by the senior executive staff Currently licensed as a Psychiatric Nurse Practitioner by the NYS Education Department; Extensive experience working with young adults (ages 18 to 25) with serious mental illness, co-occurring substance abuse, who may also be justice-involved, homeless, with limited social support; Excellent written and communication skills; Bilingual Spanish speaking preferred. This position is Flexible : This position has flexible work flexibility where for which staff have flexibility to complete their work day from home or another suitable space, typically after reporting to the office and / or engaging in field or community-based work. Job Title : Psychiatric Nurse Practitioner (part-time position)
Job Summary :
- The Young Adult Assertive Community Treatment (YA ACT) team is a multi-disciplinary team that provides evidence-based, mobile, community-based mental health treatment to promote the recovery and community integration of individuals age 18 to 25 with serious mental illnesses, high rates of co-occurring substance use disorders and high utilization of emergency / crisis services. The team also provides developmentally appropriate support to build and implement productive vocational or education plans, and will focus on enhancing necessary skills for independent living and self-management.
- The Psychiatric Nurse Practitioner, along with the team leader, has overall clinical responsibility for the delivery of high quality clinical services to the YA ACT Team consumers. The Psychiatric Nurse Practitioner provides psychiatric assessment and evaluation, psychopharmacological treatment, and coordination of consumers' health care needs.
Salary : The salary for this role $166,000 per year.
