Summary
The Tuberculosis (TB) Program is seeking to fill one full-timePublic Health Nurse/Nurse Case Manager. Public Health Nurse (PHN) provides case management services to patients with active or latent disease, care coordination, referral, and follow-up, contact investigation, medication management, and ongoing physical and psychosocial assessment to individuals and families. This PHN will support patients, community members and other healthcare providers with TB education and outreach and partner with healthcare providers to understand and improve TB related healthcare practices. The PHN may also participate in research projects that seek to improve the health and treatment of TB patients.

Job Duties

    • Provide comprehensive nurse case management to active and latent TB patients.
    • Conduct clinic, hospital and home visits to assess patient needs, living situations and provide education on TB transmission, isolation, and treatment plans.
    • Coordinate medication management and treatment adherence through Directly Observed Therapy (DOT) in collaboration with Health Outreach Aides.
    • Work closely with Disease Research Intervention Specialist (DRIS) to support contact investigations and coordinate screening and evaluation of high-risk contacts.
    • Monitor treatment progress, medication adherence, side effects, and provide timely nursing interventions as needed.
    • Provide counseling and support to address patient concerns, stigma, and psychosocial barriers that may impact treatment outcomes.
    • Collaborate with multidisciplinary team members and community providers to ensure care coordination, continuity of care, and successful treatment completion.
    • Perform electronic documentation and case management in systems including Epic and TREATS.
    • Perform phlebotomy and specimen collection as needed for TB evaluation and treatment monitoring or willingness to learn phlebotomy skills is preferred.
    • Attends community events to support TB education and targeted testing efforts, conducts outreach to healthcare providers, interviews TB patients, and coordinates and participates in community engagement efforts in the community.

Experience, Qualifications, Knowledge, Skills
Requirements:
  • Washington State Registered Nurse License in good standing.
  • Washington State license as a Registered Nurse (in good standing) and a bachelor's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.
  • Washington State Drivers' License
  • Current BLS-CPR for Healthcare Providers
  • Successful completion of background check and pre-employment physical.


Work Schedule: This is a fully benefited, 1.0 FTE Career Service position and is designated as hourly/non-exempt under the regulations of the Fair Labor Standards Act (FLSA). Typical work schedule is Monday through Friday, 8 a.m. to 4:30 p.m. and may work outside of the typical schedule to meet operational needs.
Work Location: The TB Clinic, which is located at Harborview Medical Center in Downtown Seattle. This position is also a member of a state-wide strike team and may be dispatched to other Counties across the state with short notice to provide TB support services.
This posting will be used to fill all needs. Interviews for this opportunity will be held the week of June 1-3.
