Qualifications:- Possess an Associate's or Bachelor’s Degree in Nursing from an accredited college or university.
- Possess current licensure as a registered nurse in the State of Michigan.
- One year of experience in home health care, behavioral health nursing or hospital setting.
- Must possess a valid driver’s license in the State of Michigan.
- Effective interpersonal skills to interact with staff, patients, families, outside health care providers, and the community.
Duties and Responsibilities: The RN – IDD Behavioral Health Home provides nursing care and care coordination for individuals with intellectual and developmental disabilities with complex medical and behavioral health needs. This role includes completing nursing assessments, contributing to individualized care plans, educating individuals and families on health conditions and medications, and coordinating care with medical, behavioral health, and community providers to support continuity of care. As part of an interdisciplinary team, the RN delivers authorized nursing services, applies trauma-informed principles, supports individuals in crisis or at varying stages of change, and contributes to quality, outcomes monitoring, and effective use of the electronic health record.
- Participates in the selection of strategies to implement evidence-based wellness and prevention initiatives.
- Participates in initial care plan development including specific goals for all enrollees.
- Communicates with medical providers, subspecialty providers including mental health and substance abuse service providers, long term care and hospitals regarding records including admission and discharge.
- Provides education and resources around health conditions, treatment recommendation, medications, and strategies to implement care plan goals, including both clinical and non-clinical needs.
- Provides guidance and care as an active member of the IDD Behavioral Health Home Team.
- Assures training to clinical and care coordination team members as identified by direct supervisor.
- Monitors assessments and screenings to assure findings are integrated in the care plan.
- Facilitates the use of the Electronic Health Record (EHR) and other Health Information Technology (HIT) to link services, facilitate communication among team members and provide feedback.
- Monitors and reports on performance measures and outcomes.
- Provides quality nursing services to individuals as authorized in their Individual Plan of Service.
- Meets regularly with the care team to plan care and discuss cases and exchanges appropriate information with team members in an informal manner as part of the daily routine of the clinic. Collaborates as part of an interdisciplinary team in assessing, planning, implementing, and evaluating services for individuals with psychiatric, co-morbid, and/or co-occurring psychiatric and substance use disorders.
- Skilled at listening, engaging, and responding to individuals at various stages or readiness for change; those in crisis; and those with complex issues such as co-occurring psychiatric and substance use disorders.
- Collects medical histories and records vital signs.
- Performs nursing assessments and establishes/updates care plan for individuals as needed.
- Works with referral source, family, physicians, and staff to assure continuity of care. Collaborates as part of interdisciplinary team in assessing, planning, implementing, and evaluating service needs.
- Demonstrates knowledge of principles of trauma informed care.
- Willingness to support and assist individuals with trauma related issues.
